South's 3NT bid shows long solid clubs and reasonable values outside.  You lead the spade jack and declarer wins with the ace in dummy, six and four from East and South.  Declarer then plays diamond two, three, king and it's over to you to defend.

Solution

It appears that declarer has the spade king and not the queen since she didn't attempt to run the spade lead to the queen and partner didn't play the king to show spade king and queen.  She also can only have six solid clubs or she would have cashed nine tricks.  It looks as though she has the diamond queen as well and has just set up her ninth trick.  So the defence needs to cash heart tricks quickly.  West must switch to the jack of hearts.  Success!  Partner has AK1094 of hearts and the contract goes two off.  Note that a low heart isn't good enough.
